Global sharing of experience for promoting the microbiology laboratory capabilities by France and China
1) The background of Hangzhou Forum.
2) The sharing of the construction of microbiology laboratories.
3) The importance of microbiological detection.
4) The development of the "hard power" about automation of microbiology laboratories .
5) The improvement of the ‘soft power’ about the inspection personnel of microbiology laboratories.
June 2, 2018, Hangzhou, China, hosted by the French company bioMérieux, supported by the French Healthcare Alliance, held a Sino-French Forum on the topic of "Laboratory Development and Efficiency Improvement". The personnel from the microbiology laboratories and the management of the hospital from China and France, are conducting a rigorous and professional discussion about the same points and differences, the urgency and development tendency about the construction of microbiology laboratory automation, especially reaching a consensus for the urgency of optimizing the laboratory processes and improving microbiologists’ “soft power”.
Professor Jean-François CROS, the vice director of the Hospices Civils de Lyon, said:” Against the background of an increase in bacterial drug resistance, from the point of view of improving the prognosis of patients, the rational use of antibiotics and all aspects of the health economy, it is extremely valuable to invest resources to establish a high-level microbiology laboratory”.
Professor Jean-François CROS, Hospices Civils de Lyon
According to the experience sharing by professor Jean-François CROS, the centralization of the microbiology laboratory is very common in France, the Hospices Civils de Lyon is responsible for the detection of all the microbial specimen in 14 affiliated hospitals. In France, it has already integrated the 4000 different level laboratories into 1000 center laboratories and this combination is still underway. The combination of laboratories and intensively detection of microbial specimen can promote the standardization of microbiological testing process, improve the quality of the report. At the same time, the concentration of specimens also facilitates automated processing, which can free the staff from daily repetitive work and promote the overall capacity of microbiology laboratory.
Professor Hui WANG, Peking University People's Hospital Director of Inspection, told us :”Recently, the National Health Council has committed the Notice on Continually Doing a Good Job on Clinical Application Management of Antibacterial Drugs, which stressed that the need to gradually change the clinical application management of antimicrobials from "mainly administrative intervention" to "mainly multidisciplinary and cooperative management" and to give full play to the role of clinical microbiological testing in multidisciplinary antimicrobial management, the higher requirements and expectations are placed on the capabilities of the microbiology laboratory. It is very urgent to learn from the advanced experience of foreign countries and increase investment in the development of the microbiology laboratory.
Professor Hui WANG, Director of Inspection Department, People's Hospital of Peking University
See microbiology know disease, how important is microbiological detection?
What exactly is the difference between microbiological tests and our routine inspections in hospitals? How can microbiological examination make great strides in effectively suppressing the abuse of antibiotics?
Thus, Professor Hui WANG, the leader of clinical microbiology in China, gave an exhaustive explanation : "In order to combat microbial infections, multidisciplinary efforts are needed, and microbiology laboratories can identify and confirm the pathogens of infection, which can assist clinical selection of the most effective antimicrobial agents and therapeutic schedule. The targeted treatment and personalized treatment of infectious pathogens can be achieved by the closely cooperation between microbiology laboratories and clinic. At the same time, the microbiology laboratory can discover and report to the clinic in time about the "super bacteria" resistant to multiple antimicrobial agents and is the key to cut off "Super bacteria" spreading in hospitals.
“Clinical doctors need a rapid and accurate microbiological diagnostic report. A complete microbiological report need to provide quantitative MIC test results, information on drug resistance mechanism and drug grouping, ect. Which can directly inform the clinical doctors of the infection pathogen, and which one has the higher success rate.” The professor in Hangzhou Sir Run Run Shaw Hospital Yunsong YU also pointed that : “In addition, the summary analysis of historical data also contributes to the main basis for empirical treatment at the time of infection.
Professor Songyun YU, Associate Dean and Director of Infectious Diseases, Hangzhou Sir Run Run Shaw Hospital
Professor Hui WANG supplemented: “At the moment, the cognition degree of microbiological detection is very low in clinical practice in China which caused the inspection rate largely lower than the international average. The resources we invest in the clinical microbiology are relatively low and severely unbalanced. This is the main reason why our clinical microbiology development falls behind the tests of biochemistry and immunoassays. Our microbiological personnel need: “Cohesion and prosperity”, enhancing the communication between all clinical offices, striving for all of the resources which can improve the level of microbiological laboratory, servicing for the clinic, strengthening the collaboration between all subjects, letting more patients benefit from microbiological testing.
Taking the “soft power” as a breakthrough point, to be the most reliable partner in the development of the China Microbiology Laboratory
Microbiological testing is a complex process that involves the collection, delivery, and processing of patient samples, the isolation, culture, and identification of pathogenic microbial pathogens in samples, and drug susceptibility testing, only then can an accurate report be produced. Each step in the process is related to the accuracy of the final result.
During the experience shared by the professor Jean-François CROS:” In France, in addition to the inspection equipment that can provide standardized technical support for each step, we also attach great importance to the optimization of the entire process and the technical level of the microbiological laboratory personnel, because only by ensuring that each step is standardization and normalization, and harmoniously combined, then interpreted correctly by qualified laboratory personnel, an accurate microbiology test report can be obtained.”
Professor Hui WANG agreed that:” The development of automation of microbiology laboratories does not only relay on advanced technology equipment, but also need the integrated development of ‘hard power’ and ‘soft power’ at the same time. The importance of “soft power” runs through the standardized operation of the entire inspection process including specimen collection and submission before the specimen analysis, and professional reading ability of the test report, and the timely and professional communication with the clinical aspect. Clinical microbiologists cannot simply position themselves as operators of equipment, but should become bridges between microbial infections and clinicians.”
To this regard, professor Yunsong YU expressed that: “Only the accurate testing results can support the realization of accurate therapy, lead the advanced microbiological detection technology used by the real clinic, can ultimately benefit patients.”
During the forum, experts of both sides have reached a consensus on the comprehensive automation and standardization of microbiological detection in the future unstoppable development direction. Especially in the course of automated development of Chinese microbiology laboratories, how to use the experience of French centralization and standardization to reverse the thinking of laboratory staff and hospital administrators, how to improve the overall knowledge and skills of the laboratory personnel and their ability of thinking, to continuous optimization and linking of the entire process of microbiological testing, leading the way to the transformation of microbiology laboratories in China for in-depth brain collisions.
